Good meeting notes answer three questions for someone who wasn't there: what was decided, who's doing what by when, and what's still open. These templates are built around exactly that. Copy the one that fits, paste it anywhere, delete what you don't need.
The standard template
Works for team meetings, project syncs, and client calls. The Decisions and Action items sections are the ones people will actually read later; keep Notes short and put the weight there.

The decision log
For meetings whose entire point was making a call. Recording the options you rejected is the part future-you will thank you for, because every big decision gets relitigated within six months.

What makes notes get used
The failure mode of meeting notes isn't missing information, it's burying the action items under a transcript nobody reads. Write decisions as complete statements ("We're shipping the beta March 3 without SSO", not "discussed beta timing"). Give every action item an owner and a date, because an action item without both is a wish. And send notes the same day; notes shared three days later document a meeting everyone has already misremembered differently.
